issue with worlfow to be started

Hi,

I have created a workflow to be started for email to be triggered if the PM order is created ,  I want the workflow to be started only if the plant is 5122 . I know we can restrict the condition at the below path.

Header (button) --> Version Independent task  (tab) --> start events (tab ) --> column condition  which is nothing but start condition . I want to maintain the start condition as plant 5122 and order type equal to PM01 or PM02 or PM03 , I am not sure on how to set up the condition .

Please suggest and also see attached screen shot.
